The purpose of this survey is to gather information about our OLMC ministries for the website and for one-page summaries that any new potential minister might review to help them decide whether your ministry is right for them.
Submitted forms will be sent to the office email.
If you prefer, the Church can set up an email account for your ministry with the format (ministry)@ourladycarmelvalley.org. However, you need to commit to checking that email regularly. If you need help setting up your computer or smartphone to check your inbox, we can help — send a note to netadmin@ourladycarmelvalley.org.
Photographs illustrating your ministry can be compelling. If possible, please email several photographs to the office with your ministry name in the subject line.
